Pain Treatment Planning Guide

This Treatment Planning Guide is intended to serve as a guide in the evaluation, treatment and management of patients receiving chronic opioid therapy.


Assessment

    • Comprehensive assessment including history (medical, behavioral health, and substance abuse), physical exam, and review of diagnostic studies
    • Pain assessment
    • Previous response to opioid, non-opioid, and non-pharmacologic treatment

Informed Consent

Treatment Agreement

Initiate Treatment

  • Incorporation of non-pharmacologic treatment into the overall plan of care, as appropriate, based on patient’s condition and level of function (e.g., physical therapy, exercise)
  • Establishment of treatment goals

Reassessment

Clinical Decision Making

  • Continuation of the current treatment plan if there is effective pain relief and improvement in functioning
  • Management of adverse effects
  • Modifications to treatment plan
