The Department of Social Services (DSS) offers financial incentives to primary care practices that qualify for PCMH status to help offset the costs of becoming, maintaining, and operating as a PCMH for HUSKY Health program recipients.

2020 PCMH Performance-Based Payment Program Changes

The methodology for determining your level of performance or improvement on selected health measures will be changing starting in calendar year (CY) 2020.

The new methodology provides a better representation of practices that are showing high levels of performance and/or improvement on quality measures, and does not reward practices with low performance showing no improvement. Practices with high levels of improvement or performance may potentially receive higher incentives. The performance and improvement program will be using an updated list of health measures.
