If you or a family member have recently been to the hospital or the ER, it is important to see a PCP within a week. This follow-up visit will help to keep you out of the hospital.
To help you set up your follow-up visit and to answer any questions you have about your care, one of our nurses will call you at home. Our nurses have helped many people just like you get the care they need after leaving the hospital. Our goal is to help you get back on your feet as soon as possible.
